Other Medical Forms
The school must have written permission from parents and the child’s doctor in order to administer any over-the-counter or prescription medications to a child. Any medication your child needs to take at school must be brought to the staff in the original container with the pharmacy label, including the child’s name, name of the medicine, date filled, phone number, physician’s name and directions for the exact dosage of the medication. All non-emergency medications are stored in a locked box in the office and may NOT be stored in children’s cubbies. These regulations are in compliance with the nurse practice act.
